The Main Event - The Memorial Day Challenge

Online Check In is Required. To start online check in - Click on the Online Check In Icon on the front page of the event site or on your Control Panel.

Documentation to be Uploaded into the SincSports Online Check In System.

Make sure all forms are signed before uploading.

ALL TEAMS:
- Risk Management Form (More info found at IYSA)
- Official League Roster
- Official Player Passes
- IYSA Tournament Roster (Save some time and populate the roster within Online Check In Video.)
- IYSA Communicable Disease Release of Liability
- Travel Permits (Out of State USYSA teams only)
IYSA TEAMS: IYSA Guest Form (Players from within the same club do not need a guest player form)
NON-IYSA TEAMS:
- Valid State Association or US Soccer Organization Member Rosters & Guest Player Forms
- IL Medical Release Form NEW VERSION (signed and dated after 3/1/2023)
- Form NT from Non-IYSA/USYS Teams

If your team is not a USYS or US Club team

IYSA Tournament Pass is required.

When online check in is complete, the team will print a packet that has the Approved athlete list from SincSports (One per Game)
